2. The PDFs are type-able or fillable on any computer or PDF application on smartphones. You can then print for signing or use an application for electronic-signing like Adobe or Signnow.

You can also use pdffiller.com which allows filling and signing online.


Delivery Options:

1. Hand delivery to your dental office.

2. By email if you consent to this type of delivery.

Scanning of any printed form can be done using your smartphone to make a PDF and email or fax them to your dentist’s office. JPEGs or Pictures will not be accepted.

We recommend using a scanner application like TurboScan or similar from the App Store for iPhone devices or Google Play Store for android phones. All Paperwork can be scanned in in a matter of a minute or two in one PDF.
